Here are my thoughts... 1. Develop and adhere to a consistent design-basis report format which has the necessary sections. You may already have this defined. Something like... 1.0 Purpose and Executive Summary (including reference to related memos) 2.0 Assumptions (highlight those which require some action to close-out) 3.0 Analysis Methodology and Inputs (drawing, material properties, modeling approach, relevant performance requirements and design criteria) 4.0 Results and Interpretation 5.0 Summary and Recommendations 6.0 Attachments (input files or file posting site, supporting documentation, reviewers supporting calculations and comments) 2. Issue design basis memo as a draft 3. Assign an independent reviewer to verify the accuracy of the work. For traceability, have reviewer's notes included as an attachment to the report. 4. Have author work with reviewer to close all open issues and incorporate reviewer's comments into preliminary version. Maintain reviewer's comments in attachments. Issue a preliminary version of the report with author and reviewer signatures. 5. Have central administrator (maybe the project engineer) maintain a record of the action items (assumptions requiring further effort, recommendations for further analysis, testing, etc.), assign responsible persons and track for closure. 6. Issue final version with author, reviewer and appropriate line manager signatures. 7. Distribute and file in project archives. Basically, I'd like to see the reviewer's comments included in the subject memo, and not floating around as an entirely different report.
