A safety review meeting was held on August 10 2004, with Maul electric personnel in the PPPL Safety Office Conference Room.

Following is summary. Post-meeting clarifications are also added.:
1. The sub-contractors took the General Employee training in the morning today and have been given PPPL badges.
2. Maul electric was advised that the single PPPL contact person for the whole task is Don McBride (Construction Manager/Safety Officer). All communications will be established through Don. John Semler will be the back up for Don McBride.
3. Maul Electric confirmed that Dominic Siracusa (Site Superintendent) is the single contact person and designated competent person for Maul Electric at site. Maul Electric conveyed that Steven Mazurek (Berkowsky & Associates) will be the back up for Dominic. Maul Electric will send his resume for PPPL review.
Allen Wrigley advised that as a competent person, Dominic Siracusa has to take on the spot decision on Safety issues without the necessity to contact any other individual.
(Note: Based on our letter to Maul that " Maul Electric will need to provide an overall Competent Person to be onsite during the entire project in accordance with OSHA 29 CFR 1926.20 (b)(2)" Dominic's resume has been submitted to PPPL and has been reviewed.)
4. Maul Electric was advised that any of their crew at Site can STOP work if any safety issue comes up that may result in unsafe working conditions.
5. The sub-contractors were advised that PPPL gives the top most priority for personnel safety in all aspects of the job. Keeping this in view, there will be job review meeting every day prior to the commencement of work. The meeting will be at site with Don McBride, Bill Slavin and Dominic Siracusa as participants. JHAs shall be prepared or modified by sub-contractor, as job progresses and conditions change. The JHA shall be reviewed and approved by PPPL.
6. Maul Electric conveyed that they have a very good Safety Record. PPPL advised that our aim shall be for "ZERO" - i.e. no accident to any one at site.
7. Although Lindstrom is using the OSHA guidelines for fall protection in the OSHA Steel Erection standard (1926 Subpart R - 1926.760), PPPL will enforce the standard Construction Fall Protection standards (1926.500-1926.503) where fall protection is required for all work 6 feet or more above a lower level.
Any contractual disputes/ change order and requests for information must be channeled through Larry Sutton - Sub-contract Administrator, prior to performing the activity for PPPL approval.
8. Allen Wrigley pointed out that the competent person shall ascertain Soil conditions and provide shoring necessary to avoid collapse on an ongoing basis.
9. PPPL conveyed that all the digging permit requirements shall be followed.
10. PPPL is performing several tasks at site. All the safety barricades and markings/posting must be strictly observed by the sub-contract personnel.
11. In answer to PPPL's question, Bill Sharp stated that he will be at site on a weekly basis.
12. PPPL is yet to receive the safety manuals for Raritan and Fromkin. Maul Electric stated a) they are on the way and b)In addition, Berkowsky & Associates will enforce safety provisions as described in their manual as a minimum.
12. A review meeting will be held with the excavation crew by Carl Potensky (Facilities department) prior to the issue of the digging permits.
13. Based on the discussions, the organization chart has been revised to include additional information. Please see attached.
