Hi Judy,

I have revised 002 to reflect your comments:

(1) Applicability section revised according to our discussions.  Note that SOW now under CM.
(2) Procedure:
* Step 2 - revised according to our discussions about continuation sheets and CM web.
* Step 8 - deleted electronic signature requirement.  Sending form via e-mail is adequate.  Form (Attachment 2) revised accordingly.
* Step 9 - added, "if deemed necessary" to need to call CCB.
(3) Attachment 2 (Part II) - deleted requirement for signature.

Please advise if these resolutions are okay with you.
